Concordia used the helmet design with the white "CU" logo apparently more often than not during the early 2000s, but I have seen photographs from some games during at least the 2001 and 2002 seasons in which the helmets were "blank". I would appreciate hearing from anyone who can clarify specifically when the decals were used and not used.
The helmet design used during at least 1974 and 1975 has been described as featuring white shells with a "C that was formed with broken squares in blue". The team apparently used plain white helmets for a while in 1976 until American flag decals were added to the sides.

In 2016 Dakota Wesleyan used two of the above helmet designs in the following sequence: B-A-A; A-A-B; B-A-A; A-A.
In 2015 DWU used: A-A-A; A-B-B; A-B-B; B-? (no information for game #11 at Dordt).
*1 In 2014 the large tiger logo was worn only on the left side of each helmet; on the right side, blue uniform numerals were used (as shown below).
*2 This design has numerals on its left side only; the right side features a large "tiger" logo (as shown below).

In 2015 Hastings used two of the above helmet designs in the following sequence: A-A-B; A-B-A; B-A-B; A.
*1 During the 2012 and 2013 seasons the uniform numerals were worn only on the left side of each helmet; on the right side was the traditional horseshoe logo (as on the previous design). During at least one game in 2013 (October 5 vs Briar Cliff) the horseshoe decals were absent.
*2 Uniform numerals are present on the left side only of this design; the right side features a large "bronco head" logo (as shown below)..
*3 The horseshoe logo and uniform numerals are present on the left side of this design only; the right side has the same "bronco head" logo as is present on the right side of design 'C'.

The name of this school was Midland Lutheran University prior to the 2010 football season. Midland Lutheran adopted a new helmet logo in 2005; I would need a copy of the logo artwork in order to add this helmet to the website. Below is a rather poor photo from 2005 (a close look will show that this is not the same as the logo on the helmet labelled "2001-2004" above; the word "WARRIORS" is absent from this version):
*1 MLC used these plain white helmets for at least one or two games during 2000. The team also apparently used a version of the '2001-2004' helmet logo with the colors reversed (orange "WARRIORS" with a black outline) at some point around 2000 or 2001.

Morningside did not field a football team in 2001. The team returned in 2002 as an NAIA independent team, before joining the GPAC the following year. Prior to 2001, Morningside was a member of the NCAA Division II North Central Conference for many years; the team was known as the "Maroon Chiefs" prior to 1998.
I have received a substantial amount of conflicting information about earlier helmet designs used by this team. During the mid-1980s the team apparently used maroon helmets with a logo consisting of a large "M" with the word "Chiefs" across it, and still earlier (c.1980) a design featuring a circle containing an "M" with some attached feathers was in use.
